Essential Information to Support Your Surgical Care Experience

Appointments & Scheduling

Please call our office at (520) 320-5665 for appointments. Our appointment staff will make every effort to schedule your appointment at your convenience. We ask that you let us know 24 hours in advance if you are unable to make your scheduled appointment. There is a $25.00 charge for non notification of missed appointments.

A medical emergency or unanticipated problem can cause delays in the physician's schedule, therefore delays in your appointment time. Our receptionist will advise you should this happen. We appreciate your understanding if this occurs.

Prescriptions

If you need a refill on a medication that our physicians prescribed, please call your pharmacy and have them contact us. If you need to call us for refills, requests are to be called during the hours of 8:30 am to 4:30 pm. We require 48 hours advanced notice for all refills. We do not refill prescriptions for any type of narcotic or controlled substance after office hours. Please do not leave refill request with our answering service as they will not be able to help you.

Insurance

We are contracted with many insurance companies. Please be sure to give us correct insurance information at the time of your initial call for an appointment. Our trained staff will know if it is an insurance company that we participate in. It is also important for us to know if you change insurances at any point during your care. Please have your insurance card(s) available at each appointment with us.

Referrals

Many insurance companies require referrals. It is the patient's responsibility to know if the insurance company they have requires a referral and to ensure that the proper referral has been sent before their scheduled appointment. It is suggested that the patient obtain a copy of the referral to bring to their appointment even if a copy has been faxed by the primary care physician. Feel free to call our office to check on the status of your referral. We apologize for any inconvenience but we cannot see patients without a referral and non-emergent appointments will be rescheduled.

Copays & Fees

Please be prepared to pay your co-payment at the time of your visit. Our contract with your insurance company requires us to collect your co-payment and as a member of your insurance you are required to pay the co-payment.

Our fees are based on what is usual and customary for the area. If you are paying for your own healthcare we expect payment at the time of service. All co-insurance amounts as well as co-pays are due at the time of service. Our business office is available to meet with you for any of your financial concerns. There is a $40 charge for any forms to be completed by our physicians.
